Market Liaison - Healthcare, Skilled Nursing & Post-Acute Care Full-Time | West Tennessee Market | Growing Team | Bonus Opportunity Champion Care is seeking a driven, relationship-focused Market Liaison to support our skilled nursing and post-acute care centers throughout West Tennessee, including Jackson, Humboldt, Dyersburg, Paris, Henry County, and surrounding communities . This is a field-based market development role focused on building strong relationships with hospitals, case managers, discharge planners, social workers, physicians, and other healthcare referral partners while driving admissions and census growth across our skilled nursing facilities. If you have experience in healthcare business development, hospital relations, skilled nursing admissions, case management, healthcare marketing, or post-acute care , we want to talk with you. What You'll Do Build and maintain strong relationships with hospital case managers, discharge planners, social workers, physicians, and referral partners Develop hospital, physician, and community referral sources throughout Jackson, Humboldt, Dyersburg, Paris, Henry County, and surrounding West Tennessee markets Review referrals and coordinate admissions into skilled nursing and post-acute care facilities Drive admissions and census growth through strategic outreach, relationship development, and consistent follow-up Coordinate smooth transitions of care from hospitals to skilled nursing and rehabilitation settings Partner with facility Administrators, Directors of Nursing, admissions teams, and regional leadership to support occupancy goals Identify new referral sources and business development opportunities throughout the market Represent Champion Care at hospital meetings, healthcare networking events, and community outreach opportunities Monitor referral activity, market trends, and opportunities to increase market share What We're Looking For Experience in healthcare business development, skilled nursing admissions, hospital liaison, clinical liaison, healthcare marketing, case management, or a related healthcare role preferred Existing relationships with hospitals, case managers, healthcare providers, or referral sources in Jackson and West Tennessee are a strong plus Knowledge of skilled nursing, post-acute care, rehabilitation, or healthcare operations preferred Strong communication, sales, networking, and relationship-building skills Ability to work independently and manage a multi-city territory Results-driven approach with the ability to develop relationships and turn opportunities into referrals Clinical healthcare background is a plus, but not required Ability to travel regularly throughout Jackson, Humboldt, Dyersburg, Paris, Henry County, and surrounding areas What We Offer Competitive salary Bonus opportunity Auto Stipend Company laptop and cell phone Medical, dental, and vision insurance Paid time off Supportive leadership team Career growth opportunities with a growing skilled nursing organization Join Champion Care Champion Care is a growing skilled nursing and post-acute care organization focused on providing exceptional care, developing strong healthcare partnerships, and expanding access to quality post-acute services throughout the communities we serve.
