Job Description
Post-Acute Business Development Manager (SNF, Acute Rehab, ALF & LTACH Growth Lead)
Location:
Brevard County, FL (Field-based / On-site visits across Space Coast SNFs)
Employment Type:
1099 Independent Contractor (Transition option to Full-Time W-2 with benefits after 6-month probation). This is a

position with a monthly salary.

Competitive Monthly Base/Retainer + Generous Performance Bonuses per Signed Facility / Active Census Expansion A premier medical rounding group that specializes in chronic care management is expanding its service to Skilled Nursing Facilities (SNFs), Acute Rehab, LTACH & ALF across Central Florida delivering high-touch Internal Medicine and Pain Management clinical care. Powered by a proven natively developed Healthcare technology engine, we partner with post-acute leadership to improve clinical outcomes, lower readmission rates, and optimize patient care transition. We are expanding rapidly across Brevard County and seeking a high-energy, results-driven SNF Business Development, Acute Rehab, ALF & LTACH Marketing Lead to spearhead our facility outreach, secure ongoing clinical rounding agreements, and build long-term relationships with post-acute leadership. The Opportunity If you have an established network of contacts in Brevard County post-acute care and know how to navigate SNF operations, administrators, and directors of nursing—this role offers uncapped earning potential . You will be the primary face of the practice in Brevard County.
Your focus is twofold:
speed to entry (getting us in front of decision-makers quickly) and ongoing relationship management (nurturing key facility contacts to ensure steady census expansion).
Key Responsibilities Drive Facility Acquisition:
Identify, prospect, and engage decision-makers—including SNF Administrators, Medical Directors, Directors of Nursing (DON), and Regional Operations Leads—to secure clinical rounding contracts for our physicians.
Accelerate Onboarding:
Facilitate initial meetings, contract negotiations, and seamless onboarding of our Internal Medicine and Pain Management medical rounding teams into new facilities.
Territory Strategy:
Develop and execute a targeted territory growth plan covering SNFs across Space Coast communities (Viera, Melbourne, Palm Bay, Titusville, Rockledge, Merritt Island and Beachside).
Nurture Ongoing Relationships:
Conduct regular in-person check-ins with facility leadership and case management teams to maintain high satisfaction, resolve operational friction, and support census expansion.
Market Intelligence:
Gather feedback on facility needs, clinical gap areas, and local market trends to help refine our service delivery.
Qualifications & Requirements Proven SNF Experience:
Minimum 2-3 years of direct sales, business development, marketing, or admissions experience in Skilled Nursing Facilities, post-acute care, hospice, or home health.
Local Brevard Network:
Deep, existing relationships with SNF leadership, hospital case managers, or post-acute discharge planners in Brevard County highly preferred .
Contract Closer:
Demonstrated track record of closing B2B service or clinical contracts in healthcare settings.
Self-Starter & Autonomous:
Ability to manage your schedule, drive field visits independently, and deliver measurable growth metrics.
Communication Skills:
Excellent interpersonal, presentation, and relationship-building skills.
Compensation & Growth Path Initial Contract Structure:
1099 Independent Contractor status during an initial 6-month probation period . Base +
Bonus Structure:
Competitive monthly base compensation/retainer. Tiered performance bonuses tied directly to signed facility contracts and active patient census targets . W-2
Career Pathway:
Top-performing candidates will have the direct opportunity to transition to a permanent Full-Time W-2 leadership position with full benefits package at the conclusion of the 6-month probation period.
Schedule & Work Conditions Work Location:
Field-based across Brevard County, FL.
Travel:
Regular local travel between facilities (reliable personal transportation required).
