We are seeking a motivated and relationship-driven Hospital Liaison / Business Development Director to help grow our census, strengthen referral relationships, and serve as a key connection between our facility, hospitals, physicians, discharge planners, and community partners. This position is responsible for developing and maintaining strong referral relationships while ensuring a smooth transition for patients and families from the hospital to our facility. The ideal candidate is personable, organized, responsive, and comfortable working in a fast-paced healthcare environment. Key Responsibilities Hospital & Referral Relations Develop and maintain strong relationships with hospital discharge planners, case managers, social workers, physicians, and other referral sources. Conduct regular hospital and community visits to build awareness of our services. Serve as a primary point of contact for referral partners. Follow up promptly on referrals, admissions, and potential admissions. Educate referral sources on our services, specialties, programs, and admission criteria. Identify opportunities to increase hospital and community referrals. Business Development Develop and implement strategies to increase census and qualified referrals. Identify new referral sources and community partnerships. Maintain consistent outreach to hospitals, physician offices, home health agencies, hospice providers, senior communities, and other healthcare organizations. Monitor referral trends, admission opportunities, and market activity. Participate in community events, healthcare networking events, and professional organizations. Track business development activities and referral outcomes. Admissions & Customer Experience Collaborate closely with the admissions team to facilitate timely and appropriate admissions. Help coordinate the transition from hospital to facility. Maintain communication with patients and families throughout the referral and admission process. Ensure potential residents and their families receive a positive first impression of the facility. Assist with addressing concerns or barriers that may prevent an admission. Partner with clinical and administrative leadership to ensure a smooth transition of care. Internal Collaboration Work closely with nursing, admissions, social services, therapy, dietary, and administration. Communicate referral and admission opportunities effectively with the interdisciplinary team. Participate in census and referral meetings. Provide leadership with updates regarding referral activity, market trends, and opportunities for growth. Qualifications Bachelor's degree in healthcare administration, marketing, business, social work, or a related field preferred. Healthcare marketing, hospital liaison, admissions, business development, or sales experience preferred. Experience in skilled nursing, rehabilitation, assisted living, home health, hospice, or hospital settings strongly preferred. Strong understanding of the hospital discharge and post-acute care process. Excellent communication, interpersonal, and relationship-building skills. Strong organizational and follow-up skills. Ability to work independently while collaborating effectively with a multidisciplinary team. Professional, positive, and customer-focused approach. Ability to represent the facility professionally in the community. Valid driver's license and reliable transportation for community outreach. Hospital Liaison /
