Job Description

We are seeking a dynamic and experienced leader to join our team as an Infection Control Preventionist & Unit Manager. As a key member of our healthcare team, you will have the responsibility of overseeing day-to-day patient care while ensuring our facility meets the highest standards for infection control and prevention. The role involves supervising, directing, and developing nursing staff, and working closely with the Director of Nursing to guarantee quality patient care in our memory care facility. Key Responsibilities:
Oversee and maintain the health and well-being of resident patients by coordinating daily care activities and ensuring infection control protocols are followed. Act as the main point of contact between residents, their families, and healthcare staff, ensuring clear communication and addressing any concerns. Develop, implement, and adjust patient care plans as needed, ensuring they are in line with infection control best practices. Supervise, counsel, and provide necessary discipline to nursing staff to ensure compliance with infection control standards and quality patient care. Ensure the unit complies with all federal, state, and local regulatory standards, infection control policies, and company procedures. Work collaboratively with the Director of Nursing, Nurse Practitioners, Physicians, and other healthcare professionals to provide comprehensive resident care. Conduct regular audits to assess infection control practices and patient care, making adjustments to care plans and procedures when necessary. Perform admission and discharge assessments and manage the related documentation. Monitor and support the distribution of medication and provide hands-on patient care when required. Maintain accurate medical records, including charting, evaluations, and other necessary administrative tasks. Identify and resolve procedural and infection control issues in collaboration with the Director of Nursing. Prioritize and evaluate care needs of residents to ensure optimal care delivery and safety.
Qualifications:
Must have at least a valid LPN license in the state of Ohio CDP prefered Proven experience in a supervisory or management role within healthcare, preferably in a long-term care, skilled nursing or memory care facility. Strong knowledge of infection control practices and regulatory compliance. Excellent communication skills and ability to work effectively with patients, families, and team members.
