Providence Connections is a palliative care consultation service that has a team of experts who help patients and their families find needed resources to control symptoms associated with their illness while providing compassionate support. The team of physicians, nurse practitioners, palliative care nurses, clinical social workers and chaplains work collaboratively around high-risk heart failure and other conditions with Providence Heart Institute, as well as with Providence Cancer Center. Where You'll Work Providence Connections is a Palliative Care consultation service for patients living with a serious illness. This interdisciplinary group helps patients and their families find needed resources to control symptoms associated with their illness and maximize quality of life, all while providing compassionate support. Teams of physicians, nurse practitioners, Palliative Care nurses, clinical social workers and chaplains work collaboratively with all members of patients' care teams to provide holistic care.
