Social Services Assistant

Job description

Duties:

Collaborate with patients/residents and their families to provide support and resource information. Respond to issues identified by patients/residents and their families. Facilitate patient/resident transfer within the nursing center to ensure a seamless transition and adjustment. Document patient/resident transfers and adjustments accordingly. Participate in the health care decision-making process under the direction of the Social Services Director Assist residents to and from needed outside physician appointments.

Requirements:

High School diploma required. Associate degree in human services preferred. Minimum of two years' experience in long-term care. CPR certification required.
