Papillion, NE Pay:

As a Social Services Specialist, you'll play a pivotal role in helping patients and families navigate care transitions with clarity, compassion, and confidence—from admission through discharge and beyond. What You'll Do You're part advocate, part coordinator, part problem-solver—the connective tissue of the care experience. Guide patients and families through admissions, care planning, and transitions Assess psychosocial and social service needs and develop individualized plans Collaborate with the interdisciplinary team to ensure smooth, safe transitions of care Coordinate referrals and post-acute services in alignment with regulations and patient preferences Provide education, counseling, and support to patients and families Advocate for patient rights and ensure their voice is heard in care decisions Manage documentation including assessments, progress notes, and discharge planning Address and resolve patient/family concerns with urgency and empathy Assist with navigating financial and insurance-related matters tied to care services What You'll Bring Bachelor's degree in a healthcare-related field (preferred) 2+ years of experience in social work, case management, or care transitions Experience in post-acute or acute care settings (preferred) Strong understanding of the aging population and their unique needs Ability to stay calm, organized, and compassionate in fast-paced environments Comfort using technology and managing detailed documentation A steady presence for families during emotional or complex situations What Hillcrest Offers Multiple health plan options Vision, dental, and life insurance Paid sick leave Professional development (tuition reimbursement, certifications, and more) Dayforce Wallet - choose when you get paid Gym membership reimbursement + partner discounts Why Hillcrest?
