As a Social Services Specialist, you'll play a pivotal role in helping patients and families navigate care transitions with clarity, compassion, and confidence—from admission through discharge and beyond. What You'll Do You're part advocate, part coordinator, part problem-solver—the connective tissue of the care experience.
