The Residential Service Coordinator is responsible for providing comprehensive support services to clients of the housing facility, promoting their well-being, independence, and integration into the community. This position involves working closely with clients to assess their needs, develop individualized service plans, coordinate access to resources, and facilitate the development of life skills. The Residential Service Coordinator serves as a primary point of contact for clients and collaborates with external service providers to ensure holistic support. This position requires the ability to transport clients to and from appointments, services, or community resources as needed, using a personal or agency vehicle. Essential Duties and Responsibilities include the following. Other duties may be assigned.Client Support:
Conduct initial assessments and ongoing evaluations of clients' needs, strengths, and goals. Develop individualized service plans in collaboration with clients, addressing areas such as housing stability, mental health, substance abuse, employment, education, and life skills development. Provide case management services, assisting clients in accessing community resources, benefits, and social services. Offer emotional support, crisis intervention, and conflict resolution as needed.Life Skills Development:
Facilitate life skills training sessions and workshops on topics such as budgeting, cooking, cleaning, time management, and communication skills. Work with clients to set and achieve personal goals related to independent living and self-sufficiency. Provide guidance and support in navigating daily challenges and building resilience.Community Integration:
Assist clients in accessing and participating in community activities, events, and resources. Foster a sense of community within the program, group outings, and recreational activities. Advocate for clients' rights and needs within the community and address any barriers to their integration.Collaboration and Coordination:
Collaborate with internal and external service providers, including mental health professionals, substance abuse treatment providers, employment agencies, and educational institutions, to ensure coordinated care for clients. Participate in multidisciplinary team meetings and case conferences to discuss clients' progress, address concerns, and coordinate service delivery. Maintain regular communication with property and management staff to address maintenance issues, safety concerns, and other housing-related matters.Documentation and Reporting:
Maintain accurate and up-to-date documentation of client assessments, service plans, progress notes, and other relevant records in compliance with regulatory and organizational requirements. Generate reports on resident demographics, service utilization, outcomes, and satisfaction as required.Crisis Intervention and Emergency Response:
Respond to crises and emergencies within the housing facility, following established protocols and procedures. Collaborate with emergency services, such as law enforcement and medical personnel, as needed to ensure the safety and well-being of clients.Professional Development:
Pursue ongoing professional development opportunities to enhance skills and knowledge related to residential service coordination, case management, and supportive housing. Stay informed about best practices, emerging trends, and relevant regulations in the field. Ethics and Confidentiality:
Adhere to strict ethical standards and maintain confidentiality of client information per legal and professional guidelines. Ensure the protection and privacy of all Protected Health Information (PHI) by following all agency policies and procedures. Act as a designated Mandated Reporter; Mandated Reporter includes "Duty to Warn," which includes the responsibility to notify authorities and the intended victim(s) of physical harm.Supervisory Responsibilities:

Minimum qualifications can be met in the following ways: High School diploma or GED (Required) Mental health experience (Required). Experience with persons who have substance abuse problems, preferably individuals who are dually diagnosed (mental health & substance abuse). Knowledge of Sacramento community organizations and community resources is highly desirable.Benefits:
