Conduct thorough investigations in accordance with organizational policies, state regulations, and federal guidelines. Interview staff, individuals served, families, witnesses, and other stakeholders using trauma-informed practices. Prepare detailed, objective, and timely investigative reports and supporting documentation. Collaborate with program leadership, Human Resources, and other departments regarding investigation outcomes and corrective actions. Monitor incident trends, identify root causes, and recommend quality improvement initiatives. Participate in incident review committees and follow-up planning activities. Assist in developing and monitoring corrective action plans to ensure successful implementation. Serve as a subject matter expert on incident reporting systems, documentation standards, and regulatory requirements. Provide training and support to staff on incident management processes and best practices. Maintain professional relationships with regulatory agencies and external stakeholders. Travel to program locations as needed to conduct on-site investigations and reviews. Maintain strict confidentiality and uphold the highest ethical standards in all interactions.
