Job Description

Rehabilitative Services Associate Location:
Rolling Meadows, IL Schedule:
Full-Time, Monday-Friday (9:00 AM - 4:00 PM)
Payrate:
$21.35/hr Are you passionate about empowering individuals with intellectual and developmental disabilities to live their fullest lives? Clearbrook is seeking a Rehabilitative Services Associate to join our treatment team within the Community Mental Health Center. Our programs provide opportunities for active community integration and are designed to enhance independence and participation for the individuals we serve. In this role, you will be responsible for ensuring the safety and supervision of clients at all times while providing site-based skills training and facilitating community-based activities. You will also coordinate volunteer opportunities and function as a collaborative member of the client interdisciplinary team. Key Responsibilities Participate in developing client training groups and individualized objectives while managing daily programming to establish community skills. Supervise clients to ensure safety, apply established behavior intervention strategies, and provide transportation to community sites. Maintain required documentation including attendance and therapy notes while communicating scheduling and client progress to the team. Uphold strict client confidentiality, adhere to all agency policies, and complete mandatory training and seminars.
Education & Experience:
Minimum:
High School Education or equivalent
Preferred:
Bachelor's Degree in related field
Minimum:
Has worked with individuals with mental illness
Preferred:
One year of related experience
