Program Overview Grace Works Academy is Grace Reliant's employment training division, helping individuals develop workplace skills, gain job experience, and achieve long-term employment success. This role is part of Grace Works Academy and focuses on employment mentorship and vocational development, not residential or personal care services. Job Description Grace Reliant's growing Grace Works Academy team is looking to invite a Consumer Service Provider 2 (CSP 2) to help individuals with disabilities and other barriers build skills for competitive employment and community inclusion. We exist to help people live more complete and independent lives by teaching, mentoring, and guiding individuals toward meaningful work opportunities in Missouri. In this role, you will work closely with consumers to identify and augment their skills, interests and abilities using individual and/or group experiences. These skills, interests and abilities inform the job exploration and job development activities to follow with the ultimate goal of opening a career path for each consumer. Job Responsibilities Assisting in the development and implementation of Grace Works Academy service plans or other support plans for consumers with disabilities. Communicating with parents and guardians regarding consumer progress and instructional programs. Provide ongoing support to program participants, including mentoring skill training in accordance with the individual's needs. Facilitates individual and group activities Assists with writing reports, assessments, and lesson plans Document all consumer interactions and planning actions Participates in team and department meetings Maintaining accurate records and documentation related to instructional support activites. Conduct incoming consumer assessments. Develop relationships within the communities with business owners for potential consumer employment. Traveling within a 30-mile radius of Cape Girardeau to serve consumers. Other duties as assigned.
