Job Description
Conducts initial Social Determinants of Health screenings for individuals in jail and community settings and provides orientation to Diversion Support Services. Delivers peer support services grounded in lived experience using evidence based practices, including Mary Copeland's Wellness Recovery Action Plan (WRAP) and principles consistent with Forensic Assertive Community Treatment (FACT). Provides services in office based and community based settings, including field based outreach and engagement. Utilizes agency vehicles to transport clients, when clinically indicated, to identified needs such as treatment appointments, housing resources, benefits offices, and other recovery related services. Facilitates both individual and group interventions focused on recovery planning, self advocacy, relapse prevention, and community reintegration. Demonstrates competency in age responsive service delivery and applies structured assessment skills to interpret consumer status, identify functional needs, and coordinates appropriate supports to promote stability and reduce justice system involvement.
