Customized Community Support Individual Work one-on-one with adults with disabilities. Pay $18 per hour- Employee You would assist adults with disabilities with daily living activities in the community. Ensuring the participant is meeting their community access goals and outcomes, always assuring the health and safety of the individual. Responsible for the overall supervision of all participant(s) while riding in the employee's personal vehicle, participant(s) home, and in the community. Provides assistance with toileting, personal care, eating, walking, grooming, and any other needs as necessary to ensure cleanliness, good hygiene and overall safety. Facilitates social, recreational, cognitive, psychosocial, and all other scheduled activities as planned.
EXPERIENCE
  • Prefer experience with DD Waiver clients.
  • Prefer one (1) year experience working with adults with intellectual disabilities, but will train.
EDUCATION
High School Diploma or
GED OTHER REQUIREMENTS
  • Must have reliable transportation.
  • Must have valid New Mexico driver's license, auto insurance and registration.
  • Must pass COR and fingerprint investigation, driving record, and adhere to a drug-free environment.
  • No DUI/DWI convictions in the past seven (7) years.
  • Must be able to read, write, and speak English.

JOB TITLE
CUSTOMIZED
COMMUNITY SUPPORT INDIVIDUAL
Summary:
Assist adults with disabilities with daily living activities in the community. Supervise participant's involvement in activities and assuring the health and safety of the individual. Ensure the participant is meeting their community support goals and outcomes.
Minimum Qualifications:
Two (2) years experience working with adults with disabilities (preferred). Must have a High School Diploma/GED. Must successfully pass a criminal background check. Must possess a valid New Mexico Driver's License and have proof of minimum, mandatory automobile insurance and car registration. Must be able to operate their own personal vehicle used for transportation of participants. Must be free from communicable diseases. Must not have any DUI/DWI convictions within the last seven (7) years. Cornucopia Adult & Family Services is a drug free workplace.
Responsibilities:
Responsible for the overall supervision of all participant(s) while riding in the Employee's personal vehicle, participant(s) home, and in the community. Encourage participation to attend participant's needs as required through their respective Individual Service Plan (ISP) and as indicated by daily circumstances or as instructed. Provides assistance with toileting, personal care, eating, walking, grooming, and any other needs as necessary to ensure cleanliness, good hygiene and overall safety. Must be able to safely transfer participants from chair to vehicle, assist with toileting and other duties related to transfer/ambulation; including rising from and sitting down positions. Assists with planning and development of activities during community access to ensure outcomes and action steps will be met. Facilitates social, recreational, cognitive, psychosocial, and all other scheduled activities as planned. Must be in the community with participants 100% of the time. Completes required paperwork as instructed (timesheet, daily logs, and mileage). Paperwork must be turned in to the assigned Service Coordinator for approval before sending it to the Finance Director for reimbursement. Service Coordinator is responsible for monitoring participant's allotted budgeted hours of Community Access. Must be physically able to perform all duties. Must be able to lift twenty-five (25) pounds above the shoulders. Must be physically capable to lift, carry, and move adults in a safe manner, according to inservice training. Maintains confidentiality of all participants and their families. Never leave the participant(s) unattended for any reason. Attends scheduled meetings and in-service training pertaining to client(s). Performs other duties as assigned and required to maintain daily operations.
