Provides job search assistance or training for general workplace skills. Provides referrals for workplace literacy, academic or job training programs, or direct job referrals.
Responsibilities The Case Manager/Step-Down Navigator is responsible for meeting with all eligible adult patients at Cedar Ridge at Bethany to discuss the benefits of stepdown services as part of their continuum of care treatment. The Step-Down Navigator will be responsible for communicating eligibility to key members of senior leadership and associated departments and will be responsible for routinely collecting and entering data, specific to eligibility, into Midas for trending report purposes. The navigator will have a thorough understanding of the step-down process and will coordinate between departments. Assists therapists with discharge process, develops referral sources, and performs duties in accordance with the ANA Standards of Practice for Psychiatric Mental Health Care, and facility mission, values, policies, and behavioral standards.
Requirements:
High school diploma or equivalent required At least 21 years of age, required Successfully complete a drug test Successfully pass a national background check

Bachelor s degree in related field preferred. High school diploma or equivalent required Minimum one year of case management experience required , preferably in the mental health field. Thorough understanding of the inpatient workflow process and step-down process as it pertains to case management and discharge planning preferred . Case Management Certification preferred . Valid Drivers License, required Must be computer proficient and have strong communications and analytical skills.
