Coordinates and facilitates access to services to service recipients in the Community Adult Autism Partnership Program (CAAPP). Monitors service delivery, utilization and team cohesiveness and evaluates and uses data to support the continuing need for services. Ensures that authorizations for services are obtained and revised . Establishes and maintains community-based sites to be utilized as vocational opportunities for assigned individuals. Professional Skills Regulatory & Clinical Practice Ensure compliance with agency policies, professional standards, licensing requirements, and regulatory guidelines. Assess individual needs, develop and update treatment goals and service plans, and provide clinical guidance to treatment providers. Coordinate and oversee assigned individuals' services, activities, and treatment authorizations to support quality care and funding compliance. Facilitate team meetings and maintain required documentation. Program Delivery Coordinate and oversee services for assigned individuals to ensure quality outcomes and person-centered care. Complete intake assessments, maintain client records, and ensure timely and accurate documentation in the electronic medical record system. Monitor progress, collect and analyze data, and prepare required reports and treatment updates. Provide direct service coverage when needed to ensure continuity of care. Therapeutic Environment Foster a safe, supportive, and therapeutic environment that promotes positive behaviors, personal growth, and skill development. Encourage individuals to build independence, self-control, responsibility, and healthy interpersonal relationships. Utilize daily interactions and activities as opportunities to support treatment goals and improve overall functioning. Implement therapeutic strategies that enhance participation in daily living, social, and community activities.Qualifications:
EDUCATION
Bachelor's degree in Social Work, Social Science, Vocational/Rehabilitation Education, Psychology, Education, or related field required.EXPERIENCE
Two years' experience working with adolescents and/or adults with an autism spectrum disorder. Experience with ABA instructional methods, case management and/or counseling experience preferred. Must be able to demonstrate excellent customer service. Must be able to adapt to rapidly changing demands of the position, customer needs, and the organization. Proficient computer skills, mastery of Microsoft Office Applications, experience with data collection/analyzing platforms (Radar, Pro-Filer, and/or other internal and external software applications pertinent to the position). Strong organizational, oral and written communication skills. Requires ability to work independently, prioritize, manage multiple tasks simultaneously, and adhere to established guidelines. Valid driver's license must be maintained.PHYSICAL REQUIREMENTS
Regularly requires walking, standing, stooping, bending, repetitive wrist/hand movement, and pushing/pulling (1/2 to 3/4 of the workday). Often requires sitting. May require lifting up to 50 lbs. Range of hearing and vision sufficient to operate standard office machines and communicate with co-workers and stakeholders in person and via telephone. Free of dependency and abuse of illegal or illicit drugs, alcohol, inhalants, or other chemical substances or controlled substances. Seldom drives a Devereux vehicle or van. Frequent local travel.
