Our YHDP Transition Navigator plays a critical role in supporting young adults transitioning out of homelessness, system involvement, or instability—helping them build meaningful futures through education, employment, housing, and life skills. About the Role The YHDP Transition Navigator guides and advocates for young adults by connecting them to community resources, supporting their educational and employment journeys, and walking alongside them as they work toward their goals.
