TITLE:
LEAD CARE PARTNER JOB SUMMARY
Under the direct supervision of the Administrator, the Lead Care Partner (Lead CP) is responsible for assisting with the daily operation of the Assisted Living Facility and to ensure resident safety. Assure that each resident receives the highest standard of quality care. The Lead CP shall provide general observation supervision of the residents noting any changes in health and physical abilities, noting the need for medical attention, care plan updates, or other services. The Lead Care Partner serves as Administrative Designee in the absence of the Administrator.
QUALIFICATIONS
Must have genuine interest in Senior Adult care. Previous experience in assisted living environments are highly desirable. Previous leadership experience is desirable. Enthusiastic, self-starter, and capable of using good judgment in making decisions. Must be able to communicate well with staff, residents, sponsors, and physician
EDUCATION
High school graduate desirable. Must be able to read, speak, and follow oral and written instructions.
PHYSICAL/ENVIRONMENTAL DEMANDS
Heavy physical demands involve assisting with resident transfer and ambulation, bending, stooping, stretching and long periods of standing. The Resident Assistant must be able to talk in person/telephone, hear, see, sit, walk, be continually mobile, crouch, kneel, stoop, twist, turn, pivot, climb, balance, reach, grasp, push or pull 50-75 pounds and lift or carry up to 50 pounds.
WORK PERFORMED
  • Assisting in the selection and training of new employees. Assist Administrator with documentation of training
  • Providing leadership for all staff assigned to the Assisted Living Facility
  • Assist with ensuring adequate supplies and equipment for efficient functioning of the facility
  • Making recommendations to the Administrator that could help improve the function of the community

  • Make daily assignments of resident care (Staff work schedule)
  • Assist with shift coverage as needed
  • Perform on-call duties as assigned by Administrator
  • Works closely with the resident's family in helping them to understand and accept what is happening to their loved one, what their expectations should be, and how to help deal with the changes in behavior that may occur
  • Assist Administrator to complete resident's assessment and admission procedure
  • Assist Administrator in developing and modifying Care Plans according to the residents needs, and keeping staff informed of changes
  • Daily observation of residents noting changes in health and physical abilities that may require medical intervention
  • Keep Administrator, family, and physician informed of changes in resident's condition
  • Ordering and receiving medications according to established procedure through pharmacy supplier and company policy
  • Assist Residents with medication on assigned shift and document in EMAR
  • Record medication changes in each resident's medical record and EMAR
  • Assure resident's prescribed medication are available to the resident when they are needed
  • Check all batched medications and check all EMARs monthly. Consult with pharmacy supplier to assure prompt and accurate services
  • Consult with resident's physician as needed
  • Monitor resident's BP, pulse, and weight as ordered and document. Notify administrator of any changes.
  • Assist in completing Incident Reports for all falls, injuries, or any unusual occurrences and as defined by ADPH regulations.

  • Work with Activities Coordinator in developing and conducting activities programs
  • Participate in resident activities as needed or requested
  • Serve meals/snacks and assist with cleaning of dining areas after meals.
  • Answers emergency call signal expediently
  • Other duties as assigned
