Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using proper patient transfer methods such as Hoyer lifts and manual handling techniques. Monitor patients' vital signs and observe for any changes in health or behavior, documenting findings accurately to inform care plans. Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary. Administer medications following prescribed schedules while adhering to HIPAA regulations to ensure patient confidentiality and safety. Support clients during medical appointments or community outings by providing personal transportation assistance and ensuring safety throughout activities. Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing living spaces. Follow established care plans for residents with conditions like dementia, Alzheimer's, autism, or other neurodevelopmental disorders, providing specialized memory care and behavioral support. Experience Prior experience working in home health, assisted living facilities, nursing homes, or group homes is highly preferred. Demonstrated ability to provide patient personal care assistance and support residents' independence with dignity. Knowledge of patient monitoring techniques including vital signs measurement and recognizing signs of distress or emergencies such as seizures or falls. Experience working with individuals with developmental disabilities, intellectual disabilities, or neurodevelopmental disorders is a plus. Certification in CPR and First Aid is required; additional training in hospice care or dementia/Alzheimer's care is advantageous. Familiarity with medication administration protocols and HIPAA compliance ensures safe and confidential patient interactions. Strong communication skills for effective client interaction and collaboration with healthcare teams are essential for success in this role.
