Position Summary The part-time Home Care Assessor conducts initial and follow-up assessments for prospective and current clients. This is a non-medical role focused on identifying assistance needs, documenting client preferences, evaluating the home environment, and helping develop appropriate service plans. Assignments and hours vary according to client and payer-contract needs. Responsibilities Conduct in-person client assessments and reassessments in homes and community settings. Evaluate needs related to activities of daily living, personal care, homemaker support, companionship, respite, mobility assistance, supervision, and home safety. Gather accurate information from clients, responsible parties, families, case managers, and referral partners. Prepare clear, person-centered service plans within Solace Hill's non-medical scope. Document assessments, signatures, service needs, risks, preferences, and follow-up requirements accurately and promptly. Coordinate with office staff to support caregiver matching, scheduling, service authorization, and changes in condition or need. Protect client privacy and follow HIPAA, confidentiality, safety, incident-reporting, and agency policies. Promptly escalate urgent safety concerns or needs outside the agency's non-medical scope. Represent Solace Hill professionally and compassionately during every client interaction. Qualifications Experience in non-medical home care, caregiving, client intake, case coordination, social services, or a related setting is preferred. Strong interviewing, observation, documentation, organization, and communication skills. Ability to work respectfully with clients of all ages, backgrounds, and abilities. Ability to travel to client homes in Tempe and surrounding Maricopa County communities. Reliable transportation, a valid driver's license, and current automobile insurance. Ability to use electronic documentation, email, mobile applications, and standard office technology. Must satisfy all applicable background-screening, training, and agency credentialing requirements. Bilingual skills are welcome but not required. Schedule and Compensation This is a part-time position with variable hours based on assessment volume, client assignments, and contract requirements. The posted hourly range is $24.68-$29.72; actual compensation will depend on the assignment, qualifications, and applicable contract terms.
