Job Summary The Home Care Service Coordinator is responsible for ensuring an exceptional experience for our clients and their families while coordinating ongoing care services and supporting the growth of the agency. Serve as a backup to the Home Service Coordinator to ensure continuity of operations when coverage is needed. Answer incoming office calls and assist callers with general questions, scheduling needs, client inquiries, and other requests when needed. Assist with coordinating caregiver and client schedules when providing backup coverage. Help communicate schedule changes, call-offs, and other urgent client or caregiver needs to the appropriate team member. Identify opportunities to increase, decrease, or modify services based on client needs and agency policies. Document visits and follow-up actions accurately and promptly. Help ensure clients receive consistent, high-quality service. This position combines client care coordination, relationship management, field visits, prospect follow-up, light marketing/business development, client reassessments, and quality assurance. The ideal candidate is compassionate, organized, professional, and comfortable working both in the office and in the community. This person should enjoy building relationships with clients, families, healthcare professionals, social workers, discharge planners, and community referral partners. Duties Conduct in-home consultations with prospective clients and families to understand their care needs and explain available services. Conduct routine and as-needed client reassessments to evaluate changing care needs. Conduct supervisory/client visits to monitor the quality and appropriateness of services. Communicate client changes and care concerns to the appropriate members of the care team. Follow up with prospects after consultations to answer questions and assist with the conversion process. Maintain accurate records of leads, consultations, follow-ups, and outcomes. Maintain and update department and individual Key Performance Indicators (KPIs). Support the agency's marketing and business development efforts through relationship building and community outreach. Conduct regular visits to hospitals, skilled nursing facilities, rehabilitation centers, senior communities, and other referral sources. Develop and maintain relationships with healthcare and community professionals. Assist with agency events, community activities, networking events, and educational presentations. Assist with answering phones and being a backup for our Home Care Service Coordinator. Maintain a professional presence and positive representation of the agency within the community. Work closely with Home Services Coordinators, Client Care Marketer, caregivers, management, and administrative staff. Promote a collaborative, client-centered work environment Communicate important client updates and service concerns to the appropriate team members. Participate in staff and care coordination meetings. Assist with problem-solving when client, family, or caregiver concerns arise. Requirements Ability to manage multiple priorities and meet deadlines. Team Player Comfortable conducting visits independently in the community. Professional appearance and demeanor. Ability to work independently while also functioning effectively as part of a team. Proficiency with computers, email, Microsoft Office/Google Workspace, and client management or scheduling software. Valid driver's license, reliable transportation, and ability to travel locally between client and referral locations. Bilingual English/Spanish is a plus. Previous experience in home care, healthcare, senior care, case management, client services, sales, marketing, or a related field preferred. Strong customer service and relationship-building skills. Excellent verbal and written communication skills. Comfortable speaking with healthcare professionals, families, clients, and community partners. Strong organizational and follow-up skills. $26.00 - $30.00 per hour Expected hours: 30.0 - 40.0 per week
