This role involves assisting clients with daily activities, ensuring their safety, and promoting independence while delivering personalized care tailored to each individual's needs. Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene. Monitor patients' health status by observing vital signs, medication adherence, and overall well-being. Prepare nutritious meals in accordance with dietary restrictions and preferences. Support clients with mobility using appropriate patient transfer methods such as Hoyer lifts or manual handling techniques. Maintain detailed care plans and document patient progress, observations, and any incidents accurately. Provide transportation assistance for appointments, errands, or community activities to promote independence and social engagement. Ensure a safe environment by performing regular cleaning, sanitation, and safety checks within the living space. Requirements Prior experience working in group homes, assisted living facilities, nursing homes, or home health settings is highly preferred. Knowledge of patient observation techniques, medication administration, and vital signs monitoring. Familiarity with HIPAA regulations to ensure client confidentiality and privacy. Experience providing dementia care, Alzheimer's care, memory care, or supporting individuals with neurodevelopmental or developmental disabilities is a plus. Ability to perform manual handling tasks safely and effectively; experience with Hoyer lifts or patient transfer methods is desirable. Certification in CPR and First Aid is required; ongoing training in hospice care or geriatrics is advantageous. Strong communication skills for client interaction and team collaboration. Physical ability to lift heavy objects when necessary and perform patient transfers safely.
