This role offers an exciting opportunity to provide exceptional care and support to individuals with diverse needs, including seniors, those with disabilities, and individuals requiring memory care. The ideal candidate will have experience in assisted living, home health, or long-term care environments, and possess a strong passion for enhancing the quality of life of those they serve. Your energy, attentiveness, and commitment will make a meaningful difference every day! Responsibilities Provide personal care assistance including bathing, grooming, dressing, and mobility support for clients with various needs such as dementia, Alzheimer's, or developmental disabilities. Must have prior experience. Monitor patients' vital signs and observe for any changes in health status; accurately document patient conditions and updates in care plans Assist with activities of daily living (ADLs) such as meal preparation, medication administration, and light housekeeping to maintain a safe and comfortable environment Support patient transfer methods using equipment like Hoyer lifts or other manual handling techniques while ensuring safety protocols are followed Administer medications following HIPAA guidelines and prescribed care plans; ensure proper documentation and adherence to medication schedules Provide companionship and emotional support, fostering a respectful environment that upholds residents' rights and dignity Assist with transportation needs for appointments or community outings using personal transportation methods when necessary Maintain cleanliness of the living space through regular cleaning duties aligned with infection control standards in healthcare settings Experience Candidates should have prior experience working in group homes, assisted living facilities, nursing homes, or home health environments. Experience working with individuals with autism, neurodevelopmental disorders, or intellectual disabilities is highly valued. Familiarity with patient observation techniques, dementia or Alzheimer's care, hospice support, and managing care plans is essential. Applicants must demonstrate proficiency in patient monitoring, vital signs measurement, manual handling procedures like heavy lifting or patient transfers using Hoyer lifts, and emergency response skills including CPR and first aid. Prior exposure to HIPAA regulations ensures confidentiality and compliance in all aspects of patient care. Long-term caregiving experience focusing on geriatrics or home & community care will be considered an asset. The role demands excellent communication skills for effective client interaction and teamwork within a caregiving environment committed to respecting residents' rights.
