Duties Deliver personalized patient care by assisting with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support using manual handling techniques and patient transfer methods like Hoyer lifts. Monitor patients' vital signs and observe for any changes in health status or behavior, documenting findings accurately in care plans while ensuring compliance with HIPAA regulations. Prepare nutritious meals tailored to individual dietary needs, assist with meal consumption, and ensure proper food safety practices are followed. Provide companionship and emotional support to reduce anxiety and promote mental well-being for clients experiencing dementia, Alzheimer's, or other neurodevelopmental disorders. Support medication administration under supervision or as trained, ensuring proper storage and adherence to medication schedules. Assist clients with transportation for medical appointments or community outings using safe driving practices and client communication skills. Maintain a clean and safe environment by performing light housekeeping duties such as cleaning patient areas, laundry, and ensuring safety hazards are minimized. Experience Prior experience working with individuals with disabilities, developmental or intellectual disabilities support, or senior care is highly preferred. Demonstrated ability to work in group home settings, assisted living facilities, nursing homes, or home health environments. Knowledge of patient observation techniques, patient monitoring protocols, and resident's rights is essential. Certification in CPR and First Aid is required; additional certifications such as HIPAA training or hospice care experience are advantageous. Experience working with individuals with autism or neurodevelopmental disorders enhances your ability to provide tailored care. Familiarity with care plans development and medication administration procedures ensures high-quality patient support. Ability to perform heavy lifting safely (including use of Hoyer lifts), manual handling tasks, and assist with transfers while maintaining client safety.
