Job Description:

The SLS Worker will provide one-on-one assistance to individuals in their preferred language, both at home and in the community. We are seeking support staffs to assist a 49-year-old young man who lives independently in Rancho Santa Margarita Mondays - Fridays Night shift: 11:00 PM - 8:00

AM Responsibilities:

Assist individuals in achieving their goals in line with their Individual Support Plan (ISP). Assist individuals with activities of daily living (ADLs), such as dressing, grooming, personal care as needed. Assist individual with community integration / Access • Maintain documentation of individual's progress. Ensure confidentiality of individual's information. Record case notes for each service date and support assessment, planning, and evaluation as needed. Maintain current car insurance and provide transportation for the individual. Coach individuals using an approved curriculum. Respond to law enforcement issues regarding individuals served (24-hour response if required). Represent individuals in court when applicable. Teach and instruct individuals in all areas of the curriculum. Support individuals in all areas of need per their IPP. Provide response to situations concerning individuals served.

Skills Required:

Experience working with individuals with intellectualdisability Demonstrates patience, empathy, and strong communication. Understands person-centered support and independence principles. Follows safety protocols and behavioral support plans. Maintains accurate documentation and daily progress notes. Supports daily living skills and community participation. Communicates effectively with families and interdisciplinary teams.
