Job Description

Overview We are seeking a dedicated and organized Home Care Coordinator to join our compassionate healthcare team. In this vital role, you will serve as the primary point of contact for patients and their families, coordinating in-home care services to ensure seamless, personalized support. Your proactive approach will help facilitate effective communication between clients, caregivers, and healthcare providers, promoting optimal health outcomes and patient satisfaction. This position offers an exciting opportunity to make a meaningful difference in the lives of those needing home-based medical support. Duties Coordinate scheduling of home care services by liaising with clients, caregivers, and healthcare professionals to meet individual needs. Manage patient records using Electronic Health Records (EHR) or Electronic Medical Records (EMR) systems, ensuring accuracy and confidentiality at all times. Communicate effectively with patients and their families via phone or client communication platforms to provide updates, answer questions, and address concerns. Support medical administrative tasks such as verifying health insurance coverage, processing documentation, and maintaining patient confidentiality in compliance with health regulation policies. Utilize medical terminology knowledge to accurately document care plans, service notes, and other essential records. Assist with patient management software to track appointments, service delivery, and billing information efficiently. Provide exceptional customer service by responding promptly to inquiries and ensuring a positive experience for patients and their families. Qualifications Previous experience working in a medical office or healthcare setting is highly preferred. Proficiency in Microsoft Office applications including Word, Excel, and Outlook for documentation and communication purposes. Knowledge of health regulation policies related to patient privacy and confidentiality is essential. Familiarity with medical administrative support tasks such as appointment scheduling, insurance verification, and billing processes. Experience using patient management software or EMR/EHR systems to manage electronic health records effectively. Strong understanding of medical terminology relevant to home care services. Excellent phone communication skills coupled with the ability to handle client inquiries professionally. Ability to maintain accuracy while typing and managing multiple tasks simultaneously in a fast-paced environment.
