Duties Assist clients with personal care tasks such as bathing, grooming, dressing, and mobility support using appropriate patient transfer methods like Hoyer lifts and manual handling techniques. Support residents with activities of daily living (ADLs), including meal preparation, medication administration, and light housekeeping to maintain a clean and safe environment. Monitor patient health by checking vital signs, observing for changes in condition, and reporting findings accurately to healthcare teams or family members. Provide companionship and emotional support while engaging clients in meaningful activities tailored to their interests and needs. Follow care plans meticulously, ensuring adherence to resident's rights, HIPAA regulations, and safety protocols such as first aid and CPR procedures. Assist with transportation to appointments or community outings, ensuring client safety during transfers and travel. Document all care activities thoroughly in compliance with home health standards and regulatory requirements. Requirements Proven experience working with individuals with disabilities, neurodevelopmental disorders, or in senior care environments such as nursing homes or assisted living facilities. Knowledge of dementia care, Alzheimer's care, memory care strategies, and hospice or palliative care practices. Ability to perform heavy lifting and physically assist clients using proper manual handling techniques; experience with patient transfer methods like Hoyer lifts is preferred. Strong communication skills for effective client interaction and documentation; familiarity with resident's rights and HIPAA compliance is essential. Certification in CPR and first aid; ongoing training in home health or community care is advantageous. Experience working with individuals with autism or developmental disabilities is highly valued.
