Job Summary We are seeking compassionate and dedicated Direct Care Workers to provide high-quality support and assistance to individuals with various needs, , individuals with developmental disabilities, and other health conditions. This role involves supervise individuals at job placements or volunteer sites. Ensure they stay on task, understand expectations, and have the support they need to successfully complete their responsibilities. Communication and documentation coordinate with supervisors, families, employers, and team leader. Maintain accurate notes and reports on progress, attendance, transportation, and any incidents. Responsibilities Assist individuals with Activities of Daily Living (ADLs), including toileting, and mobility support Provide medication administration following care plans and HIPAA guidelines Support with meal preparation and feeding as needed Operate patient lifts such as Hoyer lifts to ensure safe transfers; assist with mobility using appropriate techniques Maintain a clean and safe environment through regular cleaning and sanitation practices Implement individual care plans tailored to each individual's needs; collaborate with professional teams for case management Drive residents or individuals to appointments or activities when required Respond promptly to emergencies; perform first aid when necessary Engage in conflict management and behavioral support to foster a positive environment Document all care activities thoroughly in compliance with medical terminology standards Skills Experience working in assisted living facilities or work sites Familiarity with EMR systems and medical terminology required Ability to perform heavy lifting safely and operate patient lifts such as Hoyer lifts Strong communication skills for working with people with disabilities or developmental disabilities Ability to handle conflict management effectively and maintain professionalism under pressure Valid driver's license for transportation duties; clean background check required Basic first aid certification; CPR certification is a plus Compassionate attitude towards individuals' well-being; patience and empathy are essential This position offers an opportunity to make a meaningful difference in the lives of individuals requiring support while working within a professional team committed to excellence in individuals care.
