Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patient health by checking vital signs and observing for changes in condition; report findings accurately Administer medications following care plans and HIPAA regulations to ensure safety and compliance Prepare nutritious meals tailored to dietary needs and assist with feeding when necessary Support clients with mobility using proper techniques such as Hoyer lifts and patient transfer methods to ensure safety Maintain detailed documentation of care provided, patient progress, and any incidents in accordance with care plans Provide companionship and emotional support to enhance clients' quality of life while respecting their resident's rights and preferences Requirements Prior experience working with seniors, individuals with disabilities, or in assisted living or nursing home settings is highly preferred Knowledge of dementia care, Alzheimer's care, memory care, hospice care, and autism support is advantageous Ability to perform manual handling tasks including heavy lifting and patient transfers safely Certification in CPR and First Aid is required; additional training in HIPAA compliance is a plus Strong communication skills to effectively interact with clients, families, and healthcare teams Experience working with individuals with neurodevelopmental disorders or developmental disabilities is desirable Valid driver's license and reliable transportation for client outings or appointments are preferred Join us in delivering exceptional home health and community care!
