As a CSW, you will play a vital role in supporting adults with their daily living needs, medication assistance, community involvement, and overall well-being. This position is ideal for someone who enjoys building positive relationships, making a difference in the lives of others, and working as part of a supportive team dedicated to person-centered care. Position Overview The Community Support Worker is responsible for assisting adult residents (18 years and older) with medication support, promoting health and safety, encouraging community engagement and independence, and ensuring that housing environments remain safe and well maintained. Working under the supervision of the RN/Nurse Manager, the CSW provides compassionate support while helping individuals achieve their personal goals and maintain the highest possible quality of life. What You'll Do Client Support & Community Engagement Foster positive, respectful, and trusting relationships with clients. Encourage community participation, social engagement, and the development of independent living skills. Provide person-centered support while promoting dignity, choice, and self-determination. Assist clients during times of crisis and provide appropriate intervention as needed. Medication Assistance Assist with planning and preparation of medication routes as assigned. Support clients with self-administration of medications in accordance with approved training and certifications. Complete Medication Administration Records (MARs) accurately and in compliance with applicable regulations. Health, Safety & Advocacy Monitor client well-being and help maintain a safe living environment. Report crisis situations, potential concerns, incidents of neglect, abuse, or mistreatment in accordance with agency policies and procedures. Advocate for clients with professionalism, respect, and compassion when communicating with treatment providers, support teams, and community partners. Administrative Responsibilities Complete required documentation accurately and on time. Maintain confidentiality of all client information. Assist with office and program-related tasks as assigned. Maintain accurate vehicle and transportation logs. Follow all Conexio vehicle operation and safety policies. Adhere to all agency policies, procedures, and standards of conduct. Perform other duties as assigned. Qualifications Education & Experience High School Diploma or equivalent required. Valid driver's license in good standing. Minimum of three (3) years of continuous driving experience. Required Certifications CPR/First Aid Certification. MANDT Training. Assistance with Self-Administration of Medication Certification.
