Responsibilities Provide direct personal care assistance, including help with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support using patient transfer methods and manual handling techniques like the Hoyer lift. Monitor patient health by observing vital signs, tracking changes in condition, and reporting findings accurately while adhering to HIPAA regulations to ensure confidentiality. Assist with medication administration following care plans and safety protocols to ensure proper dosage and timing. Prepare nutritious meals tailored to individual dietary needs and assist with meal presentation to promote healthy eating habits. Support clients with home & community care activities, including transportation assistance for appointments or errands, and help with light housekeeping tasks such as cleaning to maintain a safe living environment. Engage in meaningful communication with clients to foster trust, understanding their needs and preferences while respecting residents' rights. Document all care activities thoroughly in compliance with healthcare standards and organizational policies, including patient observation notes and incident reports. Qualifications Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Demonstrated expertise in patient observation, dementia care (including Alzheimer's and memory care), hospice care, and neurodevelopmental disorders such as autism. Knowledge of HIPAA regulations and experience maintaining confidentiality of sensitive patient information. Skills in patient monitoring, vital signs measurement, medication administration, CPR/First Aid certification, and use of assistive devices like Hoyer lifts. Ability to perform heavy lifting safely and execute patient transfer methods effectively while supporting individuals with disabilities or developmental challenges. Strong communication skills for client interaction and teamwork within multidisciplinary care teams. Experience working with individuals with intellectual disabilities or geriatrics is a plus. Flexibility to adapt to various client needs across different settings such as home & community care or long-term facilities.
