This vital role involves providing comprehensive assistance with daily activities, health monitoring, and emotional support within clients' homes. Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene to promote independence and dignity. Monitor patients' vital signs and observe health status to identify any changes requiring medical attention. Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary. Support medication administration following care plans and ensure proper documentation in compliance with HIPAA regulations. Facilitate safe patient transfers using manual handling techniques such as Hoyer lifts and patient transfer methods. Maintain a clean, safe, and comfortable environment by performing light housekeeping tasks like cleaning and laundry. Provide companionship and emotional support, fostering a positive environment that promotes mental well-being. Support clients with mobility challenges through assisted living techniques, including the use of Hoyer lifts or other mobility aids. Document patient observations, care activities, and progress accurately to ensure seamless communication with healthcare teams. Assist with transportation for appointments or community outings, ensuring safety at all times. Experience Prior experience working in home health care, assisted living facilities, nursing homes, or group homes is highly valued. Demonstrated ability to work with individuals with developmental disabilities, neurodevelopmental disorders such as autism or dementia, and those requiring memory care or hospice care. Knowledge of patient monitoring techniques including vital signs measurement and dementia or Alzheimer's care strategies. Familiarity with medication administration protocols and care plans aligned with HIPAA standards. Hands-on experience using patient transfer equipment like Hoyer lifts or other manual handling tools is preferred. Strong background in providing personal care assistance, supporting residents' (personal needs), and assisting with ADLs in a compassionate manner. Certification in CPR and First Aid is required or highly desirable to ensure safety during emergencies. Experience working within home & community care environments supporting individuals with intellectual disabilities or geriatrics enhances your suitability for this role.
