Responsibilities Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, and personal hygiene to ensure comfort and dignity. Support patients with mobility using appropriate patient transfer methods and equipment like Hoyer lifts or manual handling techniques. Monitor vital signs and observe patient conditions to identify any changes or concerns promptly. Prepare nutritious meals tailored to dietary needs, including specialized diets for dementia or Alzheimer's care. Administer medications following care plans and ensure compliance with HIPAA regulations for patient confidentiality. Provide transportation assistance for appointments, errands, or community outings to foster social engagement and independence. Maintain detailed documentation of care activities, patient progress, and incident reports while adhering to privacy standards. Requirements Prior experience working in group homes, nursing homes, assisted living facilities, or home health environments is highly preferred. Knowledge of patient observation techniques, medication administration, and vital signs monitoring. Experience supporting individuals with dementia, Alzheimer's disease, autism spectrum disorder, developmental disabilities, or neurodevelopmental disorders. Ability to perform heavy lifting safely and utilize patient transfer equipment such as Hoyer lifts. Certification in CPR and First Aid is required; additional certifications in hospice care or geriatrics are a plus. Strong communication skills for effective client interaction and team collaboration.
