Duties Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and toileting to ensure comfort and hygiene. Support patients with mobility and transfers using manual handling techniques and equipment like Hoyer lifts and patient transfer methods. Monitor vital signs and observe patient conditions to detect changes or concerns promptly. Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary. Administer medications following care plans and ensure compliance with HIPAA regulations for patient privacy. Maintain accurate documentation of care provided, patient progress, and any incidents or observations. Provide transportation assistance for appointments, errands, or community outings to promote social engagement and independence. Skills Proven experience working in group homes, nursing homes, assisted living facilities, or home health environments. Strong knowledge of dementia care, Alzheimer's care, memory care, and hospice care practices. Ability to perform patient observation and monitoring effectively while respecting residents' rights. Skilled in assisting with ADLs, medication administration, vital signs measurement, and patient transfer methods. Familiarity with HIPAA standards to ensure confidentiality and privacy of client information. Experience working with individuals with autism, developmental disabilities, intellectual disabilities support, or neurodevelopmental disorders. Capable of heavy lifting and manual handling safely using appropriate techniques and equipment like Hoyer lifts. Excellent communication skills for engaging with clients empathetically and collaborating with healthcare teams. Knowledge of first aid procedures including CPR certification to respond effectively in emergencies. Competence in meal preparation, personal transportation assistance, home & community care experience, and patient personal care assistance.
