Duties Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene. Monitor patients' health by observing vital signs, recognizing changes, and reporting findings to healthcare professionals. Administer medications accurately following prescribed care plans while adhering to HIPAA regulations to protect patient privacy. Prepare nutritious meals tailored to individual dietary needs and assist with feeding as necessary. Support mobility and transfers using proper patient transfer methods such as Hoyer lifts or manual handling techniques to ensure safety. Provide companionship and emotional support, engaging clients in meaningful activities and maintaining open communication. Maintain detailed documentation of care provided, patient progress, and any incidents in compliance with healthcare standards. Skills Proven experience working with individuals with disabilities, developmental or neurodevelopmental disorders, including autism and intellectual disabilities support. Strong knowledge of dementia care, Alzheimer's care, memory care, and hospice care best practices. Familiarity with home health procedures such as patient monitoring, medication administration, vital signs measurement, and first aid/CPR certification. Ability to operate medical equipment like Hoyer lifts and assist with patient transfers safely. Excellent communication skills for effective client interaction and collaboration with healthcare teams. Experience working in group homes, assisted living facilities, nursing homes, or long-term care environments is highly desirable. Capable of heavy lifting and manual handling tasks required for patient transfers or mobility assistance.
