Position Summary The Community Patient Companion provides supportive, non-medical assistance to individuals with developmental disabilities receiving services through the Florida Agency for Persons with Disabilities (APD). The companion promotes independence, community participation, safety, social engagement, and achievement of the individual's personal goals while following the client's approved service plan. Essential Responsibilities Provide companionship and positive social interaction. Assist clients with participating in community activities and recreational opportunities. Encourage independence and allow the client to make appropriate choices. Assist with daily routines and activities as permitted under the client's approved service plan. Provide reminders and prompts to help the client remain focused on personal goals and scheduled activities. Assist with meal planning, grocery shopping, and simple meal preparation when authorized. Encourage healthy lifestyle activities such as exercise, walking, and other approved recreational activities. Assist with organization of personal belongings and light housekeeping related to the client's needs. Accompany clients to approved appointments, community activities, educational activities, shopping, and other appropriate outings. Promote appropriate social skills, communication, decision-making, and community integration. Maintain a safe and supportive environment. Observe and report significant changes in the client's behavior, condition, or needs to the appropriate supervisor. Complete required service documentation, including accurate notes regarding services provided and client participation. Maintain confidentiality and protect the client's privacy and dignity. Follow all company policies, APD requirements, the client's approved service plan, and applicable Florida regulations.

Pay:

$15.00 - $18.00 per hour
