Assist with patient monitoring by observing health status, vital signs, and behavioral changes, documenting findings accurately. Help clients with medication reminders and administration in accordance with care plans and HIPAA regulations. Facilitate safe patient transfers using appropriate manual handling techniques and equipment such as Hoyer lifts or patient transfer methods. Offer companionship through engaging conversation, recreational activities, and emotional support to promote mental well-being. Maintain a clean and safe environment by performing light housekeeping tasks like cleaning, laundry, and organizing personal spaces. Support residents' rights by respecting privacy, dignity, and individual preferences within assisted living or home health settings. Skills Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments. Strong knowledge of patient observation techniques, vital signs monitoring, and dementia or Alzheimer's care strategies. Familiarity with care plans, medication administration protocols, and HIPAA compliance standards. Ability to perform manual handling tasks safely using equipment such as Hoyer lifts or patient transfer methods; experience with heavy lifting is a plus. Excellent communication skills for client interaction, family updates, and team collaboration. Competence in providing personal transportation assistance and supporting clients during outings or appointments. Certifications in CPR and First Aid to ensure immediate response during emergencies; additional training in hospice care or neurodevelopmental disorders is advantageous.
