Overview We are seeking a compassionate and dedicated Care Partner to join our team in providing exceptional care and support to individuals in Assisted Living and Memory Care neighborhoods. The ideal candidate will have experience working with seniors and individuals with developmental disabilities, demonstrating a commitment to enhancing the quality of life for our residents. This role requires a blend of caregiving skills, empathy, and the ability to work collaboratively with both residents and their families. Duties Assist residents with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and meal preparation. Provide personal care and support to individuals with Alzheimer's and dementia, ensuring their comfort and safety. Monitor vital signs and report any changes in residents' health status to the appropriate healthcare professionals. Administer medications as prescribed and maintain accurate records in EMR systems. Engage residents in meaningful activities that promote social interaction and cognitive stimulation. Maintain a clean and safe environment by performing light housekeeping duties. Support individuals with disabilities through direct support services, behavior management, and personal care. Utilize equipment such as Hoyer lifts when necessary to assist residents with mobility challenges. Collaborate with team members to develop individualized care plans that meet the unique needs of each resident. Skills Experience in senior care settings such as retirement homes, assisted living facilities, or nursing homes. Knowledge of caregiving practices including memory care, long-term care experience, and home & community care experience. Familiarity with working with people with developmental disabilities and providing direct support. Proficiency in first aid techniques and emergency response protocols. Strong interpersonal skills to effectively communicate with residents, families, and team members. Ability to drive safely for transportation purposes if required by the role. Competence in cleaning tasks to ensure a hygienic living environment for residents. Understanding of behavior management techniques to support individuals with diverse needs.
