Duties Assist residents with Activities of Daily Living (ADLs), including bathing, grooming, dressing, and mobility support using patient transfer methods such as Hoyer lifts and manual handling techniques Monitor and document vital signs, patient observations, and changes in health status to ensure timely intervention and adherence to care plans Administer medications and assist with medication management following HIPAA regulations and facility protocols Support residents with meal preparation, feeding, and ensuring proper nutrition tailored to individual dietary needs Conduct patient observation for safety and behavioral changes, especially in memory care, dementia care, Alzheimer's care, and neurodevelopmental disorders Assist with patient transfers using appropriate patient transfer methods and ensure resident's rights are always respected Maintain a clean environment by performing routine cleaning duties in resident rooms and common areas in compliance with health standards Provide emotional support through effective communication, active listening, and compassionate caregiving that respects each resident's dignity Support hospice care needs by offering comfort measures and assisting family members as needed Document all care activities accurately in compliance with healthcare regulations and facility policies Experience Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred Demonstrated experience with patient monitoring, medication administration, and vital signs measurement Familiarity with dementia care, Alzheimer's care, memory care, and caring for individuals with neurodevelopmental or developmental disabilities Knowledge of HIPAA regulations related to patient privacy and confidentiality Experience working with individuals with autism or intellectual disabilities supporting their unique needs Hands-on experience with patient transfer methods such as Hoyer lifts and safe manual handling techniques Strong background in caregiving roles involving personal transportation assistance, meal preparation, light housekeeping, and assisting with ADLs Certifications in CPR and First Aid are required; additional certifications related to hospice or geriatrics are advantageous Ability to work effectively in home & community care settings while demonstrating excellent client communication skills Ability to use Hoyer Lift Join us to be part of a dedicated team committed to enhancing lives through compassionate caregiving.
