Support individuals with mobility, transfers, and ambulation. Monitor vital signs & provide medication reminders. Assist with meal planning, preparation, and feeding. Engage clients in meaningful activities and provide companionship. Maintain a clean and safe environment for clients. Communicate effectively with clients and their families. Complete daily activity logs for each shift.
