In this vital role, you will provide support with daily activities, health monitoring, and emotional well-being within the comfort of clients' homes. Your energy, empathy, and attention to detail will help promote independence and enhance quality of life for those you serve. This paid position offers a rewarding opportunity to build trusting relationships while delivering essential care tailored to each individual's needs. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and record vital signs such as blood pressure, pulse, and temperature to ensure health stability Administer medication following prescribed care plans and HIPAA regulations to maintain compliance and safety Prepare nutritious meals aligned with dietary requirements and assist with feeding as needed Support clients with mobility using equipment like Hoyer lifts or transfer belts, ensuring safety during movement Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment in accordance with care standards Observe and document changes in clients' condition or behavior, communicating promptly with healthcare teams or family members Skills Proven experience in home & community care, assisted living, nursing homes, or group homes setting Knowledge of patient observation techniques and ability to monitor vital signs accurately Familiarity with Alzheimer's care, dementia care, memory care, autism support, and hospice care practices Ability to assist with ADLs and provide direct support tailored to individual needs Strong understanding of HIPAA regulations to ensure confidentiality and privacy of patient information Competence in CPR and First Aid certification for emergency response readiness Physical capability for heavy lifting and assisting with mobility devices; stamina for long shifts Excellent communication skills for working effectively with individuals with disabilities or developmental challenges Experience working within care plans, medication administration protocols, and patient monitoring procedures Physical Therapy a + 24 hour care Embark on a fulfilling career where your compassion meets your skills—help us create safe, supportive environments that empower individuals to thrive at home.
