In this vital role, you will provide comprehensive assistance to clients in their homes, promoting independence, safety, and well-being. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and personal hygiene to ensure comfort and dignity. Monitor patients' health by observing vital signs, managing medication administration, and updating care plans in accordance with healthcare protocols. Support patients with mobility and transfers using manual handling techniques such as Hoyer lifts and patient transfer methods to promote safety. Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and preparation to foster healthy living. Provide companionship and emotional support, engaging clients in social activities or hobbies to enhance mental well-being. Maintain a clean, safe living environment by performing light housekeeping tasks such as cleaning, laundry, and organizing personal spaces. Document patient observations accurately while adhering to HIPAA regulations to ensure confidentiality and proper communication with healthcare teams. Assist with transportation needs for appointments or errands, ensuring safe and reliable client mobility within the community. Experience Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable. Demonstrated ability to observe patient conditions closely, including monitoring vital signs and recognizing changes in health status. Hands-on experience with dementia care, memory care, or supporting individuals with neurodevelopmental or developmental disabilities is preferred. Knowledge of medication administration, first aid, CPR certification, and emergency response procedures is essential for ensuring client safety. Familiarity with care plans development and implementation tailored to individual needs in geriatrics or long-term care settings is advantageous. Experience working with individuals with autism or intellectual disabilities supports our commitment to inclusive caregiving practices.
