Responsibilities Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and personal hygiene to ensure comfort and dignity. Monitor patient health by observing vital signs, administering medications as prescribed, and reporting any changes to healthcare professionals promptly. Support residents with mobility using appropriate transfer methods such as Hoyer lifts and manual handling techniques to promote safety during movement. Prepare nutritious meals tailored to individual dietary needs while maintaining cleanliness and sanitation standards in the kitchen area. Provide companionship and engage residents in meaningful activities to foster mental stimulation and emotional well-being. Maintain accurate care plans, document patient observations, and communicate effectively with team members to ensure continuity of care. Assist with transportation needs for appointments or community outings, ensuring safe personal transportation assistance. Requirements Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Knowledge of patient observation techniques, medication administration, vital signs monitoring, and HIPAA compliance is essential. Demonstrated ability to perform patient transfers using Hoyer lifts or other approved methods; experience with manual handling is a plus. Strong understanding of dementia care, Alzheimer's care, memory care, hospice care, and caring for individuals with neurodevelopmental or developmental disabilities. Certification in CPR and First Aid is required; additional certifications such as medication administration or hospice training are advantageous. Ability to work independently while maintaining excellent communication skills with clients and team members. Physical stamina to handle heavy lifting and long periods of standing or walking; patience and compassion are key traits for success.
