The ideal candidate will possess a strong background in home health care, senior care, or assisted living environments,with a focus on delivering personalized, respectful, and professional support. Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support. Monitor patients' vital signs and observe for any changes in health status or behavior, documenting findings accurately for care plans. Administer medication reminders following prescribed instructions while adhering to HIPAA regulations to ensure patient confidentiality. Prepare nutritious meals and or meal prep tailored to individual dietary needs and assist with feeding as required. Support patients with cognitive impairments such as dementia or Alzheimer's disease through memory care techniques and personalized engagement strategies. Maintain a clean and safe environment by performing light housekeeping tasks including cleaning, laundry, and organization. Provide transportation assistance for medical appointments, social activities, or errands to promote community involvement. Assist with the implementation of individualized care plans developed by healthcare professionals. Support patients during hospice or palliative care situations with compassion and professionalism. Communicate effectively with clients' families and healthcare teams to ensure coordinated care delivery. Skills Proven experience working with individuals in home health settings, assisted living facilities, nursing homes, or group homes.
