Assist residents or clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patient health by observing vital signs, reporting changes, and maintaining accurate care records while adhering to HIPAA privacy regulations Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption as needed Support medication administration following prescribed instructions and care plans under supervision or as trained Maintain a clean and safe environment through regular cleaning duties and proper infection control practices Use specialized equipment such as Hoyer lifts or other assistive devices to ensure safe patient transfers and mobility assistance Document patient progress, update care plans, and communicate effectively with healthcare teams to ensure comprehensive care delivery Proven experience in home & community care, assisted living, nursing homes, or group homes; experience working with individuals with developmental disabilities or dementia is highly valued Certifications in First Aid and CPR are required; additional certifications such as HIPAA training or hospice care are a plus Knowledge of patient observation techniques, medication administration, vital signs monitoring, and dementia/memory care best practices Ability to perform heavy lifting safely and operate assistive devices like Hoyer lifts or wheelchairs when necessary Strong communication skills with the ability to work collaboratively within multidisciplinary teams Compassionate attitude with a genuine desire to provide respectful and person-centered care Flexibility to work various shifts including evenings, weekends, or holidays as needed This role is ideal for dedicated caregivers eager to bring their skills into a supportive environment where every day presents an opportunity to positively impact someone's life.
