Duties Assist residents with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary Monitor patient health by taking vital signs, observing changes in condition, and reporting findings accurately to healthcare professionals Prepare nutritious meals aligned with dietary plans and assist with meal consumption to ensure proper nutrition Administer medications following prescribed schedules and maintain detailed documentation in compliance with HIPAA regulations Support residents with memory care needs related to Alzheimer's disease and dementia through engaging activities and consistent routines Maintain a clean and safe environment by performing light housekeeping tasks including cleaning common areas and resident rooms Follow individualized care plans designed by healthcare teams to meet each resident's specific needs and preferences Skills Proven experience in home & community care, assisted living, nursing homes, or long-term care environments Knowledge of patient monitoring techniques, vital signs measurement, and medication administration procedures Familiarity with specialized care for individuals with developmental disabilities, autism spectrum disorder, or dementia-related conditions such as Alzheimer's disease and memory care needs Ability to perform heavy lifting safely and operate assistive devices like Hoyer lifts or patient lifts Strong understanding of HIPAA regulations to ensure confidentiality of patient information Excellent communication skills for working collaboratively with healthcare teams, residents, and families Certifications such as CPR and First Aid are highly preferred; experience with hospice or home health services is a plus Join us in providing compassionate caregiving that transforms lives!
