Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and personal hygiene Monitor patient health by observing vital signs, reporting changes, and maintaining accurate care plans Support client with mobility using appropriate transfer methods such as Hoyer lifts and patient transfer techniques Prepare nutritious meals and assist with meal planning to meet individual dietary needs Maintain a clean, safe environment through regular cleaning duties and sanitation practices Provide emotional support and companionship to promote mental well-being and reduce feelings of isolation Assist with transportation for appointments or community activities using clients personal vehicle Follow all safety protocols including first aid, CPR certification requirements, manual handling procedures, and heavy lifting techniques, company will certify caregiver if they are not CPR certified Collaborate with healthcare teams to implement care plans tailored to each individual's needs, including those with developmental disabilities or neurodevelopmental disorders Ensure client's rights are upheld at all times by respecting their autonomy and dignity Qualifications Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable Knowledge of patient observation techniques and monitoring vital signs is essential Familiarity with HIPAA regulations and maintaining confidentiality in healthcare settings Ability to perform manual handling tasks safely using equipment such as Hoyer lifts and patient transfer methods Demonstrated ability to assist with ADLs and personal care assistance effectively and compassionately Experience working with individuals with autism or intellectual/developmental disabilities is advantageous Excellent communication skills for client interaction and team collaboration Ability to handle heavy lifting safely and perform patient transfers as needed Home & community care experience supporting seniors or individuals with disabilities is a plus Join our team to make a positive impact every day!
