Job Summary:
The Field Support Team Lead is a liaison between care center leadership and field employees. The role requires a high degree of collaboration with other care center team members including the hiring and onboarding coordinators, the scheduling team, field nurses and care center leadership to maintain relationships with and resolve problems for the field staff. The position will also assist care center leadership SRM/DOO in ongoing initiatives and projects.
Essential Functions:
Responsible for improving field staff engagement and retention through regular events, activities, and building strong relationships with field staff. Supports scheduling team and home care aides with service delivery to clients, families and caregivers. Functions as a coach to home care aides. Explains and reinforces job role, policy and standards of practices. Performs on-site home visit orientations, supervision visits, skills checks and annual reviews for home care aides in compliance with federal, state, and applicable accrediting body and company policies. Participates in investigations related to services complaints, dissatisfactions or incidents under direction of SRM and DOO. Supports SRM team in other areas including scheduling when necessary. Communicates concerns in employee performance to supervisor. Communicates changes in patients' health or function to appropriate nursing staff and
SRM/ DOO.
Demonstrates flexible and efficient time management, and helps team meet care center productivity standards. Performs other duties as assigned.
Knowledge, Skills and Abilities:
Experience in home care services and caregiving Displays initiative in solving day to day issues in collaboration with the SRM and DOO Demonstrated computer skills. Capable of working with data base and Microsoft office products. Strong problem solving skills and communications skills (both written and oral). Demonstrated ability to work independently with minimal direction May require evening and weekend working hours.
Qualifications:
High school diploma or equivalent. Successful completion of the Home Health Aide training or its equivalent or a current, unencumbered CNA license with successful completion of the Home Health Aide crossover training or its equivalent. Three plus years of experience in health care or related field with a current Nurse Aide license, or Home Health Aide Certificate., Supportive Home Care Aide or LMHC Experience with consumers care plans. Experience in healthcare industry a plus Licensed driver in the appropriate state with available and independent transportation and current insurance coverage.

The hiring range for this position is $26-$30/Hourly. Various factors will determine final compensation such as a candidate's years of relevant work experience, skills, certifications, and location.
