In this vital role, you will provide compassionate assistance with daily activities, health monitoring, and emotional well-being within clients' homes. Your energetic and proactive approach will help promote independence, safety, and quality of life for those in need of long-term or short-term care. Duties Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, and personal hygiene to ensure comfort and dignity. Monitor patients' health status by observing vital signs, recognizing changes, and reporting findings accurately in accordance with HIPAA regulations. Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary. Support residents with mobility using proper patient transfer methods including Hoyer lifts and manual handling techniques to ensure safety for both client and caregiver. Administer medications following prescribed care plans and document medication administration meticulously. Maintain a clean, safe, and organized environment by performing light housekeeping tasks such as cleaning, laundry, and sanitation. Provide companionship and emotional support to enhance mental well-being, especially for individuals with dementia, Alzheimer's, autism spectrum disorder, or developmental disabilities. Assist with transportation needs for medical appointments or community activities while ensuring client safety during travel. Follow all protocols related to home health care, hospice care, memory care, and resident-specific care plans to deliver consistent quality service. Qualifications Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable. Knowledge of patient observation techniques, vital signs measurement, medication administration, and first aid/CPR certification is essential. Familiarity with specialized care such as Alzheimer's/dementia care, autism support, neurodevelopmental disorders, intellectual disabilities support, and geriatrics enhances your effectiveness in this role. Ability to perform heavy lifting and manual handling safely while assisting residents with mobility or transfers using equipment like Hoyer lifts. Strong communication skills for client interaction and documentation while maintaining confidentiality in compliance with HIPAA standards. Experience in meal preparation, personal transportation assistance, patient monitoring, and implementing individualized care plans is preferred.
