Duties Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using proper patient transfer methods and manual handling techniques such as Hoyer lifts. Monitor patients' vital signs and observe for any changes in health or behavior, documenting findings accurately while adhering to HIPAA regulations to ensure confidentiality. Prepare nutritious meals aligned with dietary plans and assist with feeding when necessary, supporting overall health and well-being. Administer medications safely following prescribed care plans and maintain detailed records of medication administration. Support clients with cognitive impairments such as dementia or Alzheimer's disease by implementing specialized memory care strategies and engaging in meaningful activities. Provide transportation assistance for medical appointments, errands, or community activities to promote social engagement and independence. Maintain a clean, safe environment by performing light housekeeping duties such as cleaning, laundry, and organizing to ensure a comfortable living space. Communicate effectively with clients' families and healthcare teams to coordinate care plans and report any concerns or changes in condition promptly. Skills Proven experience working in home health, assisted living facilities, nursing homes, or group homes with a focus on geriatrics or disability support. Strong patient observation skills to detect subtle changes in health status or behavior that may require intervention. Knowledge of HIPAA regulations to protect client privacy and confidentiality at all times. Competency in patient personal care assistance including assisting with ADLs and mobility transfers using techniques like manual handling equipment such as Hoyer lifts. Ability to perform first aid and CPR confidently to respond effectively during emergencies. Experience working with individuals with autism, neurodevelopmental disorders, intellectual disabilities, or developmental disabilities supporting their unique needs. Familiarity with care plans, medication administration protocols, vital signs monitoring, dementia care strategies, and hospice or palliative care practices. Excellent communication skills for engaging with clients compassionately and collaborating with healthcare professionals and family members. Physical ability for heavy lifting and manual handling tasks required for patient transfers and mobility support. Knowledge of home & community care practices along with experience supporting seniors or individuals with disabilities in various settings.
