Begin after 30 days of employment Join Our Team Eisenhower Center is growing and seeking compassionate, dependable Direct Service Professionals to support residents in achieving their personal goals and greater independence. Are you familiar with individuals with Community Mental Health (CMH) or Traumatic Brain Injury (TBI) needs? Key Responsibilities Provide daily support and meaningful opportunities that help residents achieve their individual goals. Follow each resident's person-centered and behavioral support plans. Provide compassionate, respectful, and high-quality care while maintaining a safe and healthy environment. Build trusting relationships while protecting residents' rights, dignity, and confidentiality. Assist residents with activities of daily living (ADLs) and other support needs. Transport residents to appointments and community activities as needed. Attend appointments, advocate for residents, and obtain necessary documentation when required. Remain awake and immediately accessible when assigned to homes requiring 24/7 care. Complete Daily Progress Notes (DPN), Direct Data Sheets (DDS), and other required documentation accurately and on time. Communicate changes in resident behavior, needs, or safety concerns to appropriate staff. Follow all organizational policies, procedures, training requirements, and safety standards. Maintain a positive and professional attitude with residents, coworkers, and visitors. Arrive on time for scheduled shifts and participate in shift reports during staff transitions. Work assigned schedules consistently. Additional shifts may be available with approval. Perform other duties as assigned. Experience providing direct care and assistance with ADLs preferred. Ability to communicate effectively with residents, families, and coworkers. Qualifications High school diploma or GED. Valid driver's license and reliable transportation. Previous healthcare or caregiving experience preferred. Experience providing direct care to individuals with disabilities preferred. Ability to pass all required background screenings. Ability to follow instructions, policies, and procedures. Ability to meet the physical and emotional requirements of the position.
